Information on data protection at the State Examination Office and Recognition
- Application form completed with date and signature
- Curriculum vitae with date and signatur | fillable template (docx)
- Copy of passport, identity card or residence permit
- Copy of birth certificate + translation
- Copy of marriage certificate + translation
- Copy of diploma + translation
- Copy of certificates/ overview of grades + translation
- Copy of specialised internship (if possible with period and department) + translation
- Copy of professional examination/work licence or professional permit/registration + translation
- Copy of subject and timetable + translation
- Copy of the work certificates of your training occupation in the country of training, stating the period and name of the department + translation
The documents must be submitted in the national language and German translation - both as a copy.
- Personal declaration as to whether an application for recognition has already been submitted in Germany
- Credible representation that you will be working in Baden-Württemberg (confirmation of employment or employment contract of a potential employer)
Please refrain from using folders, envelopes and other packaging material.
- Certificate of good conduct from the home country/country of training in the original + translation
- Certificate of good conduct from Germany of document type OB (for submission to an authority),

- Original medical certificate from a general practitioner stating that you are suitable for the profession of healthcare and nursing assistant

You should submit these documents later, as they must not be older than 3 months at the time the certificate is issued.
- Original proof of German language proficiency B2 from a language school with ALTE certification as a full member (e.g. Goethe, Telc, ÖSD)
